Our team believes collaboration with referring professionals is essential when developing a patientโs initial care plan. At the point of initial intake, we invite you to share your invaluable input as we coordinate your patientโs care.
Once the proper consents are in place, our team will provide ongoing telephone or email progress reports to you and other stakeholders. This also includes consulting with you as we formulate an aftercare plan. We will also provide you with a copy of your patientโs discharge summary and continuing care plan.

Supportive Counseling for Co-occurring Disorders
